Careers pre-appointment questionnaire
This questionnaire is for members who have booked a career coaching appointment and need to complete the mandatory careers pre-appointment questionnaire.
Please ensure you research any roles, clinical areas, or courses of interest before your session if you plan on discussing these with your coach. See our career coaching page for tips on how to find information and advice on specific 汤头条污料 roles.
Important Instructions
1. Check you have filled in all fields
Please complete all fields within the questionnaire, including:
- Ticking the checkbox to confirm you've read the Careers Appointment Contract and
- Ticking the "I'm not a robot" CAPTCHA checkbox.
2. Make sure your questionnaire has been submitted.
After completing and submitting the questionnaire successfully, you should see a message saying "thanks for completing the questionnaire." You should also receive an e-mail to confirm the questionnaire has been successful. (Check your junk/spam folders.)
If this doesn't happen, it means your questionnaire did not go through properly. If you experience difficulties, please notify us without delay, as failure to complete your questionnaire will result in your career coaching appointment being auto-cancelled.

- Exploring reasons behind any decisions or hesitations
What career coaching is not :
- Telling you what to do
- Giving you advice or info, or answering queries
- Suggesting suitable roles, employers, or courses
- Trying to fix or solve your problems for you
